🧬 Fertility treatments & ART: how the journey works

Fertility treatment starts with understanding both partners' health. In Malaysia, MMC Medically Assisted Reproduction V2/2025 limits medically assisted reproduction to legally married couples. Ask a fertility specialist about suitable treatment, eligibility, costs and realistic chances before committing.

Three treatments you may hear about

  • IUI: prepared sperm is placed in the uterus around ovulation; fertilisation happens inside the body.
  • IVF: eggs are collected and combined with sperm in a laboratory, with an embryo later transferred to the uterus when appropriate.
  • ICSI: a single sperm is injected into a mature egg during IVF; it may be recommended for particular fertility problems.
  • Bring relevant medical records, a medication list and questions about itemised costs, funding or insurance. Do not assume treatment is covered.

Plan the journey together

  1. 1. Arrange assessment for both partners

The specialist may recommend semen analysis, blood tests, ultrasound or other investigations based on your histories. Fertility difficulties are not a reason to blame either partner.

  2. 2. Compare appropriate options

Discuss whether treatment is needed, why IUI or IVF is being suggested, likely outcomes, risks, appointments and costs. ICSI is not automatically better for everyone.

  3. 3. Clarify policy and consent

Donor gametes or embryos for fertilisation and surrogacy are prohibited under Malaysia's National ART Policy. Review consent, embryo storage and future decisions with the clinic before treatment.

  4. 4. Follow the treatment-specific plan

IUI involves ovulation timing and sperm preparation. IVF may involve ovarian stimulation, monitoring, egg collection, a sperm sample, laboratory fertilisation and embryo transfer. Share transport, administration and recovery support.

  5. 5. Plan follow-up and emotional support

Follow the clinic's pregnancy-testing schedule and instructions for concerning symptoms. Agree how to handle updates, time off and treatment breaks; counselling can support either partner.

Classic mistakes

  • ❌ Assuming fertility is one partner's responsibility Both partners may need assessment. Share appointments and decisions while respecting each person's consent to procedures.
  • ❌ Assuming overseas options apply in Malaysia Local policy differs. Same-sex partners should not assume marriage-based treatment access or automatic legal parentage; seek confidential advice from a qualified Malaysian lawyer about their circumstances.

Questions dads ask

Does a policy prohibition mean every arrangement is a criminal offence?

Do not treat a clinical or national policy prohibition as a blanket statement about criminal law. Ask a qualified Malaysian lawyer about legal implications, parentage or other family-building routes.
